Gemeinsamer useModalFocus-Hook: Initialfokus in den Dialog, Tab/Shift-Tab zyklisch gefangen, Fokus-Rückgabe an den Auslöser (bzw. returnFocusRef, wenn der öffnende Menüpunkt mit dem Menü unmountet). Dialoge tragen jetzt aria-labelledby auf ihre Überschrift und tabindex=-1 als Fokus-Fallback.
